The Partners HealthCare at Home Pediatric Programs provide specialized in-home, family-centered care to women experiencing high-risk pregnancies, new mothers and their infants, as well as high-risk infants, children and adolescents experiencing medical challenges.

Nurses work with each family and referring physician in developing a plan of care focused on the assessment, management, support, education and coordination of care for children.
