Partners Home Care Community Health Services

Partners Home Care offers routine adult and childhood health screening clinics at over 60 sites in eastern Massachusetts communities. Clinics include blood pressure and blood sugar screening, adult and childhood immunizations, nutritional counseling, vital sign assessment, medication review and instruction, and weight management and health-related counseling.
