Partners Home
Care values our relationship with those whom we serve. We know that
respect for privacy, especially regarding sensitive and personal health
information is the foundation of that relationship. Because we respect
your privacy interests, we operate this website by the following principles:
You may use Partners
Home Care's website without disclosing personally identifiable information.
We will not obtain such information about you unless you choose to
submit it to us. For example, if you email us your comments or suggestions,
that identifies your email address.
We have taken
reasonable steps to ensure the integrity and confidentiality of personally
identifiable information that you may provide. You should understand,
however that electronic transmissions via the internet are not necessarily
secure from interception and so we cannot guarantee the security or
confidentiality of such transmissions.
Partners Home
Care's web server collects and saves the default information customarily
logged by World Wide Web server software. Our logs contain the following
information for each request: date and time, originating IP address
and domain name (the unique address assigned to your internet service
provider's computer that connects to the internet), object requested,
and completion status of the request.
We do not use
"cookies", which are small files stored on your computer's hard drive
that are used to track personal information.
Except for authorized
law enforcement investigations or other facially valid legal processes,
we will not share any information we receive with any parties outside
of Partners.

about Partners Home Care and its affiliates. The information on the
website is written and maintained by PHC staff. This site also provides
links to the web sites of Partners HealthCare System members and affiliates
that provide health information that may be written by their respective
staff, physicians or health care providers, or contracted from outside
providers. We also provide access (links) to some external web sites
for your convenience. PHC is not responsible for the availability,
accuracy, or content of those outside sites, nor does it endorse them.
This site is
not an attempt to practice medicine or provide specific medical advice,
nor does use of the site establish a doctor-patient relationship.

In accordance

Partners Home Care, Inc. ("PHC") is registered as a Service Provider
with the United States Copyright Office. Notifications of copyright
infringement claims occurring on PHC's network should be reported
to our designated agent, as follows:

must be in writing, and must include the following information as
required by the DMCA:
