For patients and families, a gift to Partners HealthCare at Home is a personal way of saying thanks, for the trusting care provided by our clinicians who greeted you at your door, and became part of your family. For others, a gift is a way to play a valuable role in helping us build services with the highest standards of care, true commitment and the greatest compassion.

When you support the Annual Fund of Partners HealthCare at Home, your contribution has a significant impact on family, friends, and neighbors, because it supports the area of greatest need.
Celebrate a Life
Memorial and tribute gifts allow donors to honor a loved one, observe a particular occasion or say thank you to someone special. For each honor/memorial gift, PHH sends notification of the gift to the individual — family, friend, or neighbor of the remembered or honored person — that you designate. (Please include the name and address of the individual(s) you would like us to notify along with your gift.)
Notice of Tribute Gifts
Families may prefer to ask for memorial gifts through the obituary. The notice in the obituary might read, “In lieu of flowers, the family requests contributions be made in memory to Partners HealthCare at Home, Development Department, 125 Nashua St, 10th Floor, Boston, MA 02114." To ensure proper notification and dedication of these gifts, please contact PHH’s development department at 617-573-2900.
