Partners Hospice History

Partners Hospice was founded in 1980 as the Hospice of Waltham, Watertown and Belmont.

In 1986, the hospice became known as Hospice West when it merged with Waltham Hospital and then later in 1995, it changed its name to HealthCare Dimensions Hospice (HCDH).

In 1996, HCDH merged with Trinity Hospice of Greater Boston, which was established in 1989 as the first hospice to serve Boston. HCDH operated as a subsidiary of the Dana-Farber Cancer Institute from 2002 through 2006, when it grew to serve more than 100 patients a day.

In November 2006, DFCI transferred ownership of HCDH to Partners Home Care when it was renamed Partners Hospice.
