PHC Therapist Receives Anna Hughes Baker Award

Partners Home Care is pleased to announce that Milton's Susan Jenkins, a physical therapist, has received this year's Anna Hughes Baker Award established in 1997 by Milton's Ed Baker as a lasting legacy to his mother. The award honors an outstanding member of the agency's Milton Care Team who exemplifies Anna's noble and compassionate qualities. Susan was presented with her award at a reception held on October 16, 2008 at Fuller Village.
Susan began her career in home care in 1974 with the Milton Visiting Nurse Association, a founding member of Partners Home Care (PHC) Susan enjoyed working independently in the field and the rewards of helping patients recover in their home surroundings. Working in home care also gave her a flexible schedule that allowed her to enjoy raising her two sons. After a career that lasted more than forty years in health care, Susan retired this past August.
In presenting her with the award, Kathy Hernon, regional director of PHC's south region, recognized Susan for being an exceptional physical therapist. "She always provides the highest quality of care to her patients."
"Sue's belief that patients recover more quickly in the home encouraged her to come up with creative and innovative therapeutic solutions that utilized common items that were found in their home surroundings," said Hernon. "She has excellent evaluation, treatment and problem solving skills as well as the flexibility to change with the times as health care has evolved."
Kevin Mearn, Town of Milton Administrator, also presented Susan with a proclamation from the Milton Town Board of Selectman congratulating her on her retirement and thanking her for her years of service to the community.

During the ceremony, PHC's President and CEO Chris Attaya, also thanked the Milton community for its generous support of the agency's mission. "Supporters such as Ed Baker, as well as the Governor Stoughton Fund, the Seth Sprague Foundation and the Copeland Foundation, provide vital support to our mission, by helping us serve the uninsured or make valuable investments in specialty programs, such as telemonitoring and cardiac care," said Attaya. "These home-based health care programs help patients manage their illness and provide great relief to their families."
Telemonitoring provides a virtual nurse visit every day - a device in the patient's home allows them to take their vitals daily and then transmit that data to a central office that enables PHC nurses to continually monitor patient's status from afar. "This is fairly new technology that is improving people's lives and becoming a mainstay in home health care."
"The Copeland Family Foundation has shared our vision for how this technology can enhance people's lives," said Attaya. "We are grateful for their generous grants that have funded this program over the past few years. With their continued support, we will be able to offer telemonitoring services to 30 Milton residents this coming year."
