Partners Home Care Admission Criteria

Partners Home Care accepts patients when it is determined that the patient's needs are appropriate for home care, the resources of the agency are appropriate to meet the needs of the patient, and the minimum financial criteria are met.

All patients receive an admission assessment that determines if the patient meets Partners Home Care's admission criteria prior to being accepted for service. This admission visit will be scheduled within 24-48 hours of referral unless the patient's condition warrants otherwise.

Partners Home Care will notify the referral source and/or patient of the admission decision within 48 hours from the time the referral is received.

The following criteria are required for admission:

• The patient must be under the care of a licensed physician, osteopath, podiatrist or dentist who is willing to accept medical responsibility for the skilled home care needs of the patient including the provision of written orders to direct that care.

• The request for service is based on a skilled health care need.

• The patient and or support system are able, capable, and willing to assist in the patient's care, when necessary.

• The range and nature of the care required by the patient can be met by the patient and/or patient's support system with intermittent support from Partners Home Care professional and paraprofessional staff.

• The patient resides in the Partners Home Care geographical service area and the agency has the resources to meet the needs of the patient.

• There is a reasonable expectation that the patient's health care needs can be safely met in the place of residence.

• The home situation ensures reasonable physical/emotional/psychological safety of staff while providing care to the patient (i.e., free from threat of physical, verbal or emotional abuse).

In the event that Partners Home Care's certified services are not suitable for the patient, consider a referral to TLC Nursing and Lifeline at Partners Home Care.
